Adopt-A-Child Sign-Up Form:
Thank you for all you do to support our GJS Community! Here is another opportunity for you to share the spirit of giving by helping to make the holidays special for a family in our community. If you and/or your organization, club, church affiliation, etc., are able to adopt a family, we are asking you to commit to purchasing an outfit and a toy for a child/family (approximately $50). Adopters will be sent the child’s gender, age, sizes, toy ideas, and hobbies. Want to help but can’t commit to adopting a child? You can donate toward gifts too! Donations will be combined to purchase gifts! Again, thank you for your continued support.
Discretionary Medication Form: If you would like the school nurse to provide some over-the-counter medicines, like Tums or Acetaminophen, to your child when needed, you need to submit this form.
